This is the VA Enrollment Certification Request Form.

This form should be completed each semester to indicate all of the courses you intend to take.
We will use the information provided on this form to certify your benefits. You can also use this form to request certification for prior semesters. If you have any questions please send an email to or call the Office of Registration and Records at 301-369-2313.
If this is your first semester or term at Capitol please make sure that you have provided the VA School Certifying Official with your Certificate of Eligibility (COE), Notice of Basic Eligibility (NOBE), or VR&E Authorization form.
