Institutions of higher education must make available to current and prospective students information about institutional policies regarding vaccinations.  

Availability of Capitol Tech's Vaccination Policy

Capitol Technology University's vaccination policy is published annually in the Guide to Residence Life. The guide is distributed to all residential students each year and is also available on the Residence Life tab of MyCapitol, which is accessible to the public.

Capitol Technology University Vaccination Policy Revised 2/26/2016

New and continuing residential students are required to submit updated immunization forms to the Office of Residence Life. These forms include emergency contact information and medical history to assist medical personnel in case of an emergency. The forms and meningitis waiver are available on the Residence Life tab of MyCapitol.

Immunization Requirements

  1. Residents born after 1956 must have two doses of the measles vaccine and one dose each of mumps and rubella. These may be administered as a single MMR vaccination. Immunizations must be verified by a medical professional.
  2. All residents should have received a tetanus and diphtheria immunization within the last 10 years.
  3. All residents must sign a meningitis waiver indicating they have received and reviewed information about the disease and either declined vaccination or provided verification from a medical professional that they have been vaccinated.

Exemptions

Residents requesting an exemption from required vaccinations due to a medical condition or religious reason must provide appropriate documentation from a medical professional or religious leader.

Residents requesting an exemption must also submit a letter describing the symptoms of the illnesses and the risks associated with contracting the disease.

All documents must be submitted prior to residential student check-in to ensure housing eligibility.
