Welcome to the official Immunization Form for Capitol Technology University Residence Life!

This Immunization Form is a REQUIREMENT for all Licensees (Residents) of Innovators Hall. Before occupancy, all residents assigned to live in Innovators Hall MUST complete the Immunization Form.

Capitol Technology University Department of Residence Life requests the following information from residence hall students. There is no medical facility on the campus; however, this information serves as a document that can be shared with health officials in case of an emergency. If there are any questions about this form or requirements for admission into the residence hall, please contact the Office of Residence Life at 240.965.2488. Please return this form along with your Housing Application Agreement to be eligible for on-campus housing at Capitol Technology University. 

Per Section 8.A of the Annual Year Housing License:

"Licensee must fulfill mandatory immunization requirements, as outlined by Office of Student Life (OSL), to live in the Facilities."
